Service and repairs of Rotary hydraulic lifts at 1600 McKinley Avenue and 1333 Fields Avenue, Columbus, Ohio on an as needed and requested basis. Currently 1600 McKinley Ave has 31 lifts, and 1333 Fields Ave has 14 lifts. Equipment consists of Rotary models: MOD30 2 post; MOD30 3 Post; and Parallelogram. Service includes (for McKinley Avenue and Fields Ave) one (1) inspection and one (1) certification per year for each lift. Parts to be included, as needed: Cover Plate #FD67BK Link Pin #FDG7113ZN Chain #FE70-48ZN Other equipment or services may be requested by the Facilities Manager or Director. This work shall be done on a time and material basis. The Requestor of work, on a case-by-case basis, shall provide to the Service Technician a description of the scope of work to be performed. The Service Technician must provide requestor a written estimate for requested work prior to commencement of work for all projects.
