Each vendor is to provide a telephone number with answering machine or voice mail to enable the state to get in touch with the vendor. Changes in telephone numbers should be given as soon as possible. Vendor must provide FEIN. Vendor should provide a minimum of three references to include company, address, telephone number, and contact person. Vendor shall furnish all labor, material except as noted below, and equipment to maintain the facilities. Paper towels, toilet tissue, hand soap, and trash can liners will be furnished by the state. Cleaning service is to replace each item as needed and notify the state if materials are needed. Janitorial service to be completed each Wednesday and Friday after 5:00 p.m. The cleaning service representatives must be employed by the vendor or designated as their authorized representatives. The vendor shall secure the keys to the building. Any key that is lost, missing, or stolen shall be reported to the department supervisor immediately. If the vendor should lose the key or if it is stolen, the cost of changing locks or keys will be deducted from the cleaning service invoice. Should the janitorial service be cancelled for any reason, keys are to be turned in within 24 hours. Invoice to be submitted at the end of each month in arrears. CLEANING SERVICES TO BE COMPLETED EVERY WEDNESDAY AND FRIDAY AFTER 5:00 P.M: • Breakroom floor will be damp mopped. • Counter/tabletop and sinks will be kept clean, and fronts of kitchen cabinets should be wiped down. • Lobby floor will be damp mopped. • Damp mop tile floors throughout facility. • Vacuum entrance mat and replace in entranceway after floor has dried. • Sweep area outside of front entrance door. • Floor in front office is to be cleaned with laminate floor cleaner. • Hallway areas and other carpeted floors in open areas, upstairs and downstairs, are to be vacuumed. • Any offices that are open are to be vacuumed. • Wastebaskets will be emptied; all obviously soiled or torn liners will be replaced throughout facility. • Trash is to be hauled out to the dumpster each service night. • There are four restrooms, two upstairs and two downstairs, that are to be cleaned each service date. All toilets and urinals will be cleaned and disinfected, including outer surfaces to include the outer surfaces on the sides, bottom and back. Sinks, faucets and countertops will be cleaned and sanitized. Wet mop bathroom floors using a germicidal detergent solution, and all mirrors will be cleaned. WEEKLY CLEANING SERVICE: • Tile floors are to be wet mopped with cleaning solution once a week.
