Specifications include, but are not limited to: The Bidder shall comply with the Facility policies and procedures and resident care policies. 4.1.2. The Bidder shall provide resident-focused outcome-driven services in accordance with applicable requirements of federal and state laws and any third party payers. 4.1.3. The Bidder shall provide the Facility with copies of cunent licenses and/or registrations and renewals for all employees of contracted provider who provide services at the facility prior to initiating services. The cunent Certificate of Liability Insurance shall be on file at the Facility at all times. 4.1.4. The Bidder shall deliver evidence-based therapy services to residents, while adhering to professional standards and treatment plans prescribed by the resident's provider or qualified therapists. The Bidder shall collaborate with facility staff by providing clinical expertise to enhance quality resident outcomes. 4.1.5. The Bidder is required to maintain sufficient staff to provide rehabilitation services for residents as directed by physician orders. 4.1.6.The Bidder shall provide supervision from a qualified therapist to qualified personnel which may include a physical therapist assistant (PTA), or an occupational therapy assistant (OTA) when furnishing services. 4.1.7. The Bidder is responsible for promptly conducting resident evaluations and reevaluations, as needed, as well as conducting home evaluations.
