Specifications include, but are not limited to: Physical Therapy Services to the residents of Jackie Withrow Hospital 4.1.1.1 Physical Therapist must provide in-house Physical Therapy Services to the residents of Jackie Withrow Hospital. This is currently 55 but could increase to around 65. *Please note that this is not a skilled care facility.* 4.1.1.2 Physical Therapist must review/evaluate all new admissions at least once annually and whenever needed throughout the consultation process. 4.1.1.3 Physical Therapist must recommend restorative treatment therapy for residents identified in need, then develop treatment care plan for the resident on a restorative plan on a monthly basis and re-evaluate residents on restorative equipment every quarter. 4.1.1.4 Physical Therapist will recommend adaptive equipment and/or seating devices. 4.1.1.5 Physical Therapist will work with restorative staff and assist with hot pack therapy treatment. 4.1.1.6 Physical Therapist must re-evaluate all residents with a mental retardation diagnosis annually. 4.1.1.7 Physical Therapist must be available for consultations needed for care conference meetings. 4.1.1.8 Physical Therapist must be available upon request in the event of any survey (OHFLAC) issues that may arise. 4.1.1.9 Physical Therapist must provide staff-inservices to the Program Service and Nursing Departments when necessary. 4.1.1.10 Physical Therapist must attend any trainings as required by compliance and be submitted for a background check through WV Cares. 4.1.1.11 Physical Therapist must document in the resident’s Electronic Medical Record at the time services are rendered...
