Specifications include, but are not limited to: A. Select one or more of the following cancers to target: breast, cervical, and/or colorectal. B. Identify and describe the clinic site(s), jurisdiction(s) served, and the overall patient demographics. C. Establish activities and processes to provide patient navigation for cancer services: 1. Provide patient reminders. Conduct a minimum of two, but preferably more, contacts with each patient. 2. Reduce structural barriers (e.g., modify administrative processes; assist with appointment scheduling, transportation, translation, or childcare; arrange alternative screening sites or hours). 3. Reduce patients’ out-of-pocket costs (e.g., provide vouchers, reimbursements, reduction in co-pays, or adjustments in insurance coverage). 4. Provide one-on-one or group education to inform patients’ understanding of cancer and cancer screening. 5. Track patient progress in completing screening, diagnosis, and treatment. 6. Link patients to other health, community, and social services, as appropriate, to address social determinants of health. D. Attend monthly and as-needed meetings with the Contract Monitor.
