Specifications include, but are not limited to: Funding bids should focus on developing impactful activities to complement MDH's statewide tobacco control efforts and should strive to maximize the reach and impact. Goals are in: • Preventing initiation among youth and young adults. • Promoting quitting among adults and youth. • Eliminating exposure to secondhand smoke; and • Identifying and eliminating tobacco-related disparities. Project Expectation: Organization must demonstrate the ability and resources to: 1. Access the population designated to be served by the project proposal. 2. Make clear action plans and track measurable outcomes to be reported as required. (Refer to Narrative, Expenditures & Performance Measures Report form to assist developing accurate performance measures plan). 3. Be fiscally responsible with receipts or documentation how the funds were spent. 4. Attend Tobacco & Cancer Coalition meetings and report on projects to the membership at the end of year meeting. 5. Submit the required (2) semi-annual Performance Reports during the fiscal year. Report due dates are January 15, 2025 for activities from July 1, 2024 – December 31, 2024 and July 15, 2025 for activities from January 1, 2025 – June 30, 2025.
