Specifications include, but are not limited to: 1. Survey Preparation. The Contractor shall use a secure, online system for survey administration (i.e., Qualtrics). Data collection must be fully anonymous. MCA will provide the Contractor with survey questions (approximately 40 questions or 10-15 minutes to complete). The Contractor shall program and test the survey. Prior survey engagement has been robust in both waves, with far greater than the minimum required number of completed surveys submitted within days of the survey opening, however, the Contractor shall propose a modest participant incentive to encourage survey completion. Note: MCA will obtain IRB approval for the survey administration. 2. Survey Administration. The Contractor shall launch the survey and monitor responses. Target recruitment goal is 10,000 completed responses (approximately 10% of the patient population, similar recruitment to prior waves). Once recruitment goals are met, the Contractor shall notify MCA and close the survey. 3. Post-Survey Administration Tasks. The Contractor shall clean the dataset (remove bot responses, incompletes) and conduct analyses including descriptive statistics, comparisons of key variables to prior survey waves, and selected new analyses requested by MCA. The Contractor shall deliver participant incentive(s) within ten (10) business days of the close of the survey. The Contractor shall meet with MCA to discuss the format for the survey report as well as data materials.
