Specifications include, but are not limited to: The Alaska Housing Finance Corporation (AHFC) is seeking proposals from an agency experienced in providing a comprehension program, which will result in improved self-sufficiency for elderly people and people with disabilities in assisted housing developments in Fairbanks, Alaska as described in the Request for Proposals (RFP). Assign a Service Coordinator to carry out the Agreement program deliverables. If appropriate staff is not currently employed by CONTRACTOR, CONTRACTOR must recruit, hire, and train staff to carry out the specified Agreement program deliverables. The Asset Supervisor and Program Administrator may provide input to CONTRACTOR about Service Coordinator candidates during the recruitment and selection process. All staff providing services under this Agreement will have criminal background checks performed consistent with CONTRACTOR’s protocols. No person may be employed or volunteer under this Agreement if they are listed on a State or Federal sex offender registry or have even been convicted of a sex offense. Provide Service Coordination/Case Management activities for 40-hours each week. CONTRACTOR shall post and maintain regular office hours at each program location. CONTRACTOR will provide alternate coverage on days that the Service Coordinator is absent due to personal leave, sick leave, or family medical leave. CONTRACTOR will provide coverage if the Service Coordinator position becomes vacant. PROGRAM DELIVERABLES 1. Individual Case Management Activities a. Develop and distribute a Welcome Packet to all new residents. b. Offer to complete an Independent Living Needs Assessment (ILNA) for each new resident upon move in. The ILNA will be offered/updated annually. If the resident refuses to participate in the Service Coordination Program, the Service Coordinator will document the refusal using the SC22 Refusal of Recommended Services or Assistance and place in the resident’s file. Forms involved in the ILNA, for both new residents and annual updates for existing residents are as follows: i. SC01 Confidentiality Agreement ii. SC02b Consent to Release Information to AHFC iii. SC04 Resident Intake iv. SC08 Service Management Plan v. SC06 Resident ADL Status c. Assist residents in identifying barriers that adversely impact their ability to live independently. d. Complete SC14 Emergency Medical Information and provide a copy to the resident and provide “File of Life” for the resident to maintain in their unit that would provide first responders with vital information in the event of an emergency. e. Act as an information source for residents and provide third party referrals to residents as needed. f. Report suspected cases of abuse, neglect, or exploitation of a resident, in writing, to Adult Protective Services (APS), Alaska Division of Senior Services, or any other appropriate agency. Maintain copies of all reports and follow-up as appropriate. g. Report all potential lease violations and all incidents that involve the safety and security of residents to the Asset Supervisor. 2. Service Coordination Activities a. Maintain Professional Service Coordinator Certification with American Association of Service Coordinators (AASC) by completing required Continuing Education Units (CEU) through the designated methods. b. As needed, meet with Asset Supervisor and/or Program Administrator to discuss goals, services, and resident concerns. c. Evaluate the effectiveness of the program and provide suggestions for program development and growth. d. Participate with periodic teleconferences with Program Administrator and other Service Coordinators located at other AHFC sites statewide to discuss common goals, program strategies, and ways to grow the program. e.
