Specifications include, but are not limited to: Lane County Department of Health & Human Services is seeking proposals from providers to conduct street outreach to engage with people living in unsheltered locations, such as cars, parks, abandoned buildings, encampments and on the streets. Encampment- Focused Street Outreach is low barrier, targeted, strategic engagement with residents of encampments with a primary goal of connecting to permanent housing. Encampment-Focused Street Outreach occurs with one encampment at a time. This program will include housing focused case management, connection to the Coordinated Entry System, and utilizing Diversion strategies to identify both traditional and creative solutions to connect individuals to housing. Lane County will only contract with contractors directly providing direct services. Sub-contracting of said services by the contractor is strictly prohibited.
