Specifications include, but are not limited to: A. Components of the Annual Occupational Medical Evaluation of members – NFPA 1582-18, 7:4. 1) Blood and Lab Testing utilizing 1582-18, 7.7.1, 7.7.2 as a guideline. Blood and lab tests shall be performed annually and include the following: a) Hemoccult Test b) Lipid Panel c) Diabetes Tests (Hemoglobin A1C and Glucose) d) Complete Blood Count e) Comprehensive Metabolic Panel f) Thyroid Panel g) PSA (men) h) Ovarian Cancer CA-125 (woman) i) Testosterone Blood Test j) Urinalysis shall include dipstick analysis for glucose, ketones, leukocyte esterase, protein, blood and bilirubin. Microscopic analysis for RBC, WBC, casts and crystals if indicated by dipstick analysis results. 2) Audiology Testing – 1582-18, 7.7.3. Audiology testing shall compare audiogram results obtained during the yearly evaluations with baseline and subsequent test results. Standard threshold shifts shall be corrected for age as permitted by OSHA. Hearing thresholds shall be assessed annually in each ear with following frequencies: a) 500 Hz b) 1000 Hz c) 2000 Hz d) 3000 Hz e) 4000 Hz f) 6000 Hz g) 8000 Hz 3) Vision Testing. Ishihara, color and depth perception. 4) Electrocardiograms, ECGs, 1582-18, 7.7.6. A resting 12-lead ECG shall be performed as part of the baseline medical evaluation and shall be performed annually as per 7.7.6.1. Evaluations shall compare baseline to subsequent ECGs as per 7.7.6.2. 5) Cancer Screenings – 1582-18, 7.7.13 through 7.7.20. Contractor shall annually inform member of the heightened risks of cardiovascular disease and various types of cancer associated with firefighting. Along with medical screening, Contractor shall provide ultrasound imaging to include, at a minimum: a) Carotid arteries b) Echocardiogram – heart with function. c) Aorta and aortic valves d) Thyroid e) Internal organs – liver, pancreas, gall bladder, kidneys and spleen f) Bladder g) Testicular and prostate – Men h) Ovaries and uterus – Women 6) Physical Exam and Consultation (1582-18, 7.6). The annual health screening shall include each of the following: a) Vital Signs b) Head, eyes, ears, nose, and throat (HEENT) c) Neck d) Cardiovascular e) Pulmonary f) Breast g) G.I. h) Genitourinary i) Hernia j) Lymph nodes k) Neurological l) Musculoskeletal m) Skin (include cancer screening) n) Vision B. Fitness Evaluation utilizing NFPA 1583 guidelines: 1) Muscular strength & endurance evaluation. a) Grip strength as specified in the 2013 NFPA 1582 Annex C.2.1.5 b) Bench press or push up test and static plank evaluation. c) Flexibility evaluation using a sit & reach box or tape measure to assess lower body flexibility. 2) Diet and Nutritional Recommendations. 3) Personal Exercise Prescription & Recommendations.
