• Complete all necessary maintenance, repairs, and alterations to the facility before any program intakes or placements to ensure the health and safety of all program staff and participants. All interested applicants will have an opportunity to schedule an appointment to walk through the facility during the procurement process. • FAC shall serve as access points into the local Coordinated Entry System and provide the following: o Standardized Homeless Verification Letter.s o Vulnerability Index – Service Prioritization Decision Assistance Tool, known as VI-SPDAT, which is a survey administered to individuals and families to determine risk and prioritization; and o Assistance to become document ready. Documents may include birth certificates, social security, identification cards, and verifying disabling conditions and chronicity for chronic homeless households. • FAC Program service shall be provided in a manner that is consistent with Housing First (HF) approach. • FAC operation staff funded under this opportunity shall only include positions that align with the HF approach. • FAC shall use a low barrier approach to admissions and shall not have pre-conditions for entry such as program fees, sobriety or completion of a treatment program and shall not solely deny entry into its program based on any previous discharge. • FAC shall request approval from HPO to ban any Participant from entering low barrier housing premises, and/or from receiving continued services. • FAC staff shall be trained and use assertive engagement, harm reduction, and trauma informed care. • FAC will provide vacancies to HPO daily. • FAC must remain open for participants to access 24 hours a day, seven (7) days a week. • FAC will use the Service Prioritization Decision Assistance Tool, known as SPDAT, as the standardized assessment and on-going assessment to identify and address participant’s needs. • FAC shall develop housing plans using a standardized template based on information obtained from the SPDAT. • FAC shall develop a crisis plan and a discharge plan during intake. • FAC shall develop expectations for participants instead of program or house rules. • FAC shall use Critical Time Intervention (CTI), a time-limited evidence-based practice. • FAC shall subcontract with Housing Innovation for consultation and training of CTI. • FAC will include Rapid Re-Housing and Homeless Prevention components.
