Specifications include, but are not limited to: The Department of Minnesota IT Services (MNIT), partnering with the Department of Human Services (DHS), intends to enter into an agreement, for up to 24 months, with Natalis Outcomes LLC. The agreement covers software licensing, maintenance and support, and assessment fees for HDI Connect for Mental Health and Substance Abuse Treatment Services (MHSATS), a division of DHS. Once implemented, HDI Connect will provide health questionnaires for clients/patients via a software-based platform. The questionnaire identifies problems with a client/patient’s level of morale, behavioral health symptoms, and impairments, and measures seven symptom domains and three impairment domains. A report is generated from the client/patient’s responses and the information can be displayed on a dashboard. The information that is provided from HDI Connect can also be displayed in different ways to effectively communicate with a variety of clients/patients. The questionnaire can also be readministered upon signs and symptoms of clinical change or at termination or completion of care to help chronicle the effectiveness of care. This allows providers to inform treatment by data and evidence, which is required by the recently revised standards published by The Joint Commission (TJC).
