Specifications include, but are not limited to: The County of Hennepin, State of Minnesota (“County”) is soliciting proposals for developmental disabilities (DD) community program coordinated services for the Human Services and Public Health Department (“HSPHD"). HSPHD has historically provided funding to community-based agencies who work directly with adult residents with developmental disabilities and related conditions to promote selfreliance and integration into their community. These resources were developed to fill in gaps that federal, state, and other sources of funding did not fully or adequately address in County communities. The priority population served by this RFP includes adults with developmental disabilities and related conditions living in community settings who do not have access to Home and Community Based Waiver services, County-funded day services, or Semi-Independent Living Skills (SILS) service. Individuals must be a Hennepin County resident and likely to have participated in special education, have had an Individualized Education Plan (IEP) in school, or a psychological evaluation that identifies cognitive limitations. This Request for proposals (RFP) solicits proposals to support adults with developmental disabilities and related conditions to achieve the highest level of self-reliance possible and integrate into their communities. Proposers would offer a wide range of innovative and creative support services that help individuals achieve their goals that may include, but not limited to, individual support, group therapy/training, social/recreational activities, and transportation.
