Washington County Disability, Aging and Veteran Services (DAVS) is accepting letters of interest from Agency Partners to provide Evidence Based Health Promotion at various sites throughout Washington County. Evidence Based Health Promotion Programs; program options • Living Well with Chronic Conditions, and / or, • Living Well with Chronic Pain, and / or, • Diabetes Self Management Program, and / or, • Tomando (Living Well with Chronic Conditions). Partnering options: • Option 1: Partnering agency will provide a space for classes take place. o DAVS will provide trained staff to pre-register, greet, gather documentation, facilitate the program and collect data. o Agency partner will market the class and recruit individuals to participate with the on site program. • Option 2: Partnering agency will provide space and staff to facilitate program. o DAVS will pay for the cost of partnering agency staff and / or designees to be certified as master trainers for afore mentioned Health Promotion Programs. o Designees will then facilitate classes on site. o Designees will recruit participants and market the program, pre-register, greet, gather documentation, facilitate the program, document and collect data. • Option 3: Partnering agency will provide space and previously certified staff to facilitate program. o Partnering agencies that have staff who are currently certified in the afore mentioned classes will facilitate classes. o Designees will recruit participants, market programming, pre-register, greet, gather documentation, facilitate the program, document and collect data.
