Specifications include, but are not limited to: 3.1 BCDHP received the National Cardiovascular Health Program Grant CDC-RFA-DP-23-0004 (2304) and the Innovative Cardiovascular Health Program grant CDC-RFA-DP23-0005 (2305). The overall purpose of the CDC Cooperative Agreements are to support state investments in implementing and evaluating evidencebased and evidence-informed strategies to prevent and manage CVD. In addition to focusing on comprehensive efforts to identify and respond to healthcare disparities and improve health outcomes, special emphasis will be placed on the prevention and control of hypertension and high cholesterol. 3.2 ADHS has the capacity to fund this project for the full five (5) year grant period if results are favorably measurable, with deliverables starting in Year 2 of the grant award period following the Grant award. Followon years will follow the State fiscal year of June 29th through June 30th timetable provided the funds are appropriated by the CDC. 3.3 ADHS has the right to partially award based on the needs and areas being services. 3.4 ADHS intends on awarding funds as follows: 3.4.1 National Cardiovascular 2304 Strategy 3B ADHS will identify one (1) Subrecipient to create or expand a (CHW) led community-clinical linkage partnership/program in one of the following counties: Apache, Cochise, Coconino, Gila, Graham, Greenlee, Mohave, Navajo, Pima, Pinal, Santa Cruz, Yavapai, or Yuma. The Subrecipient shall work with the CCL partnership/program to identify and train a team of CHWs to recruit for and implement the CCL partnership/program, providing a continuum of care and services which extend the benefits of clinical interventions and address social services and support needs leading to optimal health outcomes. Number of Awards: 1 Subrecipient Award Annual Amount: $100,000 3.4.2 Innovative Cardiovascular 2305 Strategies 3A and 3B ADHS will identify one (1) contractor to create or expand a community-clinical linkage partnership/program within Maricopa County. The Subrecipient shall work within a CCL partnerships/programs to identify and train dedicated CHWs (or their equivalents) through a CHW SDOH referral system to create and enhance community clinical links to identify social determinants of health {(SDoH) e.g., housing, transportation, access to care, and community resources} and respond to the individual social services and support needs within each county, providing a continuum of care and services which extend the benefits of clinical interventions and address social services and support needs leading to optimal health outcomes.
