Please note: This is a Sole Source Notification. Specifications include, but are not limited to: For Health Promotion services the SUBRECIPIENT shall: 1. Provide health education on navigating the U.S. healthcare system to refugee adults arriving to the U.S. between October 1, 2021 and September 30, 2022 to promote self-efficacy in order to initiate individuals on the path to self-sufficiency. a. Administer the DEPARTMENT provided pre-assessment to at least 95% of unduplicated eligible refugee adults who arrived to the United States or whose date of certification was on or after October 1, 2021; b. Report attendance of at least 95% of unduplicated eligible preassessed refugee adults to at least one class of the Navigating the U.S. Healthcare System workshop series; c. Report attendance of at least 60% of unduplicated eligible preassessed refugee adults to 100% of classes offered in the Navigating the U.S. Healthcare System workshop series; d. Administer a post-assessment to at least 95% of unduplicated eligible pre-assessed refugee adults who completed 100% of classes offered in the Navigating the U.S. Healthcare System workshop series; e. Show an increased knowledge and understanding of how to navigate the U.S. healthcare system for at least 75% of eligible unduplicated newly arrived refugee adults who completed 100% of classes offered in the Navigating the U.S. Healthcare System workshop series, a pre-assessment and a post-assessment; and f. Provide instruction during Navigating the U.S. Healthcare System classes, pre-assessments and post-assessments in a language or multiple languages that at least 95% of participants will understand. 2. Promote health literacy, wellness, medical access and disease management among recently arrived refugees through hands-on, interactive health workshops focused on specific health topics and populations. a. Administer a pre-assessment to no less than 95% of unduplicated eligible newly arrived refugee participants who arrive at the start of each health workshop; b. Administer a post-assessment to at least 95% of unduplicated eligible newly arrived refugee participants who stay for the entire duration of each health workshop; c. Show an increased knowledge and understanding of the topic covered during each health workshop of no less than 75%; d. Provide instruction in a language or multiple languages that will be understood by no less than least 95% of participants will; and e. Create and submit a health education curriculum, preassessment, and post-assessment for each health workshop to the program contact for approval, no less than one month before the material is used in workshops...
