Specifications include, but are not limited to: Applicants should address how they will accomplish each of the following requirements in the scope of work and provide a timeline for completion. Under the contractual agreement, the family-based organization will: 1. Provide mentorship and support services to families with a DHH child identified through newborn screening. 2. Provide monthly mentorship service data in a method selected to the ADPH as required by HRSA. 3. Ensure parent or deaf mentors complete a certified training course targeted at infants and children with hearing loss. 4. Provide travel reimbursement to at least one family member of a DHH child and at least one parent mentor to attend the National Annual EHDI Meeting (dates and location to be determined). If the conference is held virtually, provisions will be made for remote participation if applicable. 5. Submit invoices for travel reimbursement with support documentation to include meeting agenda, itemized travel receipts, travel itinerary, and other requested documents as required per state travel policy and auditing purposes. 6. Submit monthly invoices for reimbursement with support documentation to include hours worked for salary and fringe by the required deadline or closeout date.
