1. Contractor shall provide, pick up, transport, dispose of refuse, and properly reposition the following: 2. Contractor shall provide one, three yard container picked up one time per week at the Cedar City Court and one, four yard container picked up one time per week at the Department of Natural Resources at the service locations listed above. Containers are to remain on-site for the duration of the contract. 3. This contract will include delivery of the container, pickups, transport and disposal of refuse. 4. Landfill charges are not included in this contract. An itemized account will need to be presented, along with a letter stating the increases. Fuel surcharges and franchise fees will not be paid. 5. Payments shall be made in monthly installments. Contractor billings shall be submitted to DFCM within five (5) business days following the end of each month.
