Specifications include, but are not limited to: The County is unable to provide a succinct program description as this is a request for innovation. The goal is to improve access to same-day care while decreasing system costs. All proposals that provide cost-effective, rapid start-up, and relieve current bottlenecks are welcomed. Each proposal will be assessed for its ability to improve access to care and reduce inappropriate levels of care. This is an open request for proposals with an indeterminate cost. The County retains the right to issue more than one contract in response to this RFP and the funding available to it ($3.2MM to be expended by 12/31/2025). Please note that the information and data designed to assist respondents in creating proposals is contained in the Addendum posted this same date, that references RFP #21325, provision of services funded by HB 4136. Respondents are not limited to the suggestions raised by the Stakeholder group in that Addendum. REQUESTED SERVICES: The County requests proposals that: 1. Produce positive, quantifiable results within the first 6 months 2. Reduce the use of inappropriate ED services 3. Improve access to same day or next day care 4. Provide and promote cost-effective care 5. Innovatively address physical health, behavioral health, or social needs. 6. including reducing unnecessary ambulance transportation
