Specifications include, but are not limited to: The Department of Health and Senior Services (DHSS), Bureau of Cancer and Chronic Disease Control (CCDC) requires a contract to deliver the Epidemiology and Laboratory Capacity Federally Qualified Health Centers (FQHCs) Initiative to coordinate the Community Health Worker Initiative (CHWI) through FQHCs. The CHWI allows Community Health Workers (CHWs) to identify underserved populations and match them with resources to assist with education, surveillance, referrals to programs and other resources in efforts to mitigate COVID-19. Missouri Primary Care Association (MPCA) is recognized by the Health Resources and Services Administration and the Bureau of Primary Health Care as the only Primary Care Association in the State of Missouri that assists FQHCs in their mission to provide high quality comprehensive health care to the medically underserved. Through Missouri FQHCs, MPCA has established the only non-profit statewide quality and performance improvement program, Missouri Quality Improvement Network, that provides support for FQHCs in leveraging Health Information Technology (HIT) and population health management software to improve patient care and reporting on clinical quality measures. Additionally, MPCA provides statewide electronic population health data aggregation tools that connect to the FQHC electronic health records and has a data use agreement in place with the FQHCs that governs the use of the clinical and demographic data collection and reporting. Furthermore, MPCA is the only organization with the resources, networks with FQHCs, and expertise to assist in increasing the number of CHWs in FQHCs assisting in the referral process to increase the number of participants screened for Social Determinants of Health (SDOH) and referrals for COVID-19 Disparities.
