Specifications include, but are not limited to: Shankle PSH is a Housing First, permanent supportive housing program serving chronically homeless households. The program should serve 11 households without children (singles) in a scattered site, community-based setting. Shankle PSH is a Continuum of Care (CoC) Leasing program in which the agency will secure scattered site units for use by the program and enter into subleases with program participants. Using a Housing First model, the goal of Shankle PSH is to serve individuals who are chronically homeless with a goal of successfully moving them into permanent housing, and further supporting the individuals in establishing and maintaining stability in permanent housing, by providing supportive services and increasing self-sufficiency, as well as independent living skills. Case managers will assist individuals in locating and securing housing, building positive relationships with landlords and neighbors, developing communication and other living skills, establishing an action plan to increase income through employment and/or benefits assistance, obtaining mainstream benefits and connections to community-based resources. All individuals served in the Shankle program will have one or more disabilities that impact their ability to live independently. As appropriate, individuals should be offered or connected to healthcare resources including but not limited to physical, mental and behavioral health providers, primary care providers, and health insurance options. All participants in the Shankle PSH program will be referred through coordinated Entry or the Central Wait List, as established by the Lane County CoC. This project is required to track enrollments, services, and client level data in the homeless Management Information System (HMIS).
