Specifications include, but are not limited to: The Kentucky Department of Fish and Wildlife Resources requires the services of physicians, nurse practitioner or physician assistants licensed to practice medicine in Kentucky or authorized to practice medicine in accordance with the rules and regulations of the US Armed Forces following an actual physical exam. The Contractor must: Be knowledgeable of the requirements set forth in the Peace Officer Professional Standards Medical Screening Guidelines Implementation Manual Form G-3 to assist the contractor in making the qualification determination. The contractor should be familiar with the responsibilities of a Certified Peace Officer. See Attachment F- Form G-3 Medical Screening Guidelines for further review of these requirements. Provide a complete physical to include a Visual Acuity Screening, Audiogram, Cardiac Risk Assessment, EKG (only if indicated by history or if resting pulse is less than 50 or greater than 100), Urinalysis and Tuberculosis Skin Test. The Medical History Statement Form G2, will be completed by the applicant and provided to the Contractor for review prior to the physical. See Attachment G - Form G-2 Medical History Statement. The Kentucky Law Enforcement Council Medical Examination Report-Form G-1 will be completed by the Contractor post-examination and provided back to the KDFWR Law Enforcement Division within 3-5 working days. See Attachment H - Form G-1 Medical Examination Report. The Department of Criminal Justice Training Basic Training Branch’s Basic Training Medical Confirmation Form 45 will be signed off on by the contractor and returned to KDFWR Law Enforcement Division within 3-5 working days. See Attachment I - Form 45 Basic Training Medical Confirmation. The contractor must have multiple locations across the state and be able to accommodate our timeline by either offering walk-in appointments or priority scheduling.
