Specifications include, but are not limited to: 2.1.1. Offerors are required to complete and pass the Arizona Mandatory Control Baseline Prerequisite Assessment (https://azdohs.gov/sites/default/files/arizona_baseline_security_controls_prerequisite__0_0.xls) and submit as part of their Offer. Arizona Department of Homeland Security (AZDOHS) Cyber Command will evaluate and rank submitted Arizona Baseline Infrastructure Security Controls for completeness, attention to detail, and understanding of National Institute of Standards and Technology (NIST) security controls and framework. AZDOHS Cyber Command will forward assessment results and recommendations to the Procurement Officer. Results of these IT security control reviews are for internal use only and will not be shared with responding bidders, though the results may be used by the Procurement Officer to determine Offeror susceptibility for award. 2.1.2. Pursuant to State Data Classification Policy P8110, and the Data classification defined in the Special Terms and Conditions, the Contractor will have access to the following type of Data as part of its work under the Contract: ☐ Low Impact Data (AZRAMP 125 / NIST Low Impact) ☐ Moderate Impact Data (AZRAMP 325 / NIST Moderate Impact) 2.1.3. Provide qualified and trained staff to collect and assess information for the health and well-being of all Veteran Residents. 2.1.4. Work with the interdisciplinary team (IDT) to schedule, plan, and manage MDS completion. 2.1.5. Review and ensure MDS’s are completed within RAI guidelines by all the interdisciplinary team (IDT). 2.1.6. Ensure care plans are initiated upon admission and for every MDS. Ensure that the goal dates are updated every three months per RAI guidelines. 2.1.7. Set assessment reference dates (ARD) and complete clinical items. 2.1.8. Complete care area assessment (CAA) for items triggered by MDS. 2.1.9. Ensure each participant completes there triggered CAAs 2.1.10. Provide a Registered Nurse (RN) signature of CAA Completion 2.1.11. Provide case mix index (CMI) management, capture planned or current resources and schedule ARDs. 2.1.12. Provide patient driven payment model (PDPM) review and management. 2.1.13. Provide Omnibus Budget Reconciliation Act and long term care review and management. 2.1.14. Provide daily chart review for Veteran/Resident changes and management.
