Clearly defined procedures for test ordering that include but is not limited to: 2.2.5.2.1. Criteria for unacceptable specimens. 2.2.5.2.2. Criteria for the reinfection of specimens. 2.2.5.2.3. Criteria for ensuring that unacceptable specimens are not run/tested. 2.2.5.3. Instructions for properly completing the Laboratory Test Request form that include but is not limited to: 2.2.5.3.1. ICD-10 coding 2.2.5.3.2. Third party payer information (Medicare, AHCCS, ALTCS, etc.) 2.2.5.3.3. Patient’s identification number. 2.2.5.3.4. Follow-up on incorrectly completed request forms. 2.2.5.3.5. Procedure for the receipt of improperly completed laboratory test request form. 2.2.5.4. Reference manual that includes but are not limited to the following topics: 2.2.5.4.1. Lab tests available. 2.2.5.4.2. Lab groupings (order by group vs. individual test). 2.2.5.4.3. Lab fees. 2.2.5.4.4. Color of tube per test or specific container. 2.2.5.5. Written policies and procedures for correcting inappropriate/compromised specimens. 2.2.6. Provide the necessary laboratory supplies for a successful collection and transportation of laboratory specimens to ASVH staff. This includes but is not limited to the following: 2.2.6.1. Microbiology suppliers 2.2.6.2. Containers, tubes, cups, etc. 2.2.6.3. Slides 2.2.6.4. Phlebotomy supplies 2.2.6.5. Forms to include but not limited to the following: 2.2.6.5.1. Clinical requisitions 2.2.6.5.2. Cytology requisitions 2.2.6.5.3. Surgical requisitions 2.2.6.5.4. Supply requests 2.2.6.5.5. Phone reports 2.2.6.5.6. Specimen labels.
