Specifications include, but are not limited to: • Develop an evaluation and performance management plan that describes: o The type(s) of evaluations to be conducted (e.g. process or outcome evaluations); o The frequency that evaluations will be conducted; o How applicant will: Analyze data that support and report on the performance measures; and Collect and analyze data that correspond to the evaluation questions, o Engage key program partners (e.g. public health, education, out-of-school time) to participate in the Evaluation Services; o How performance data will be reported; and o How quality of performance data will be assured. • Responsible for drafting the evaluation and performance measure documents for the CDC Reports o Draft of Annual Evaluation Plan; o Final of Annual Evaluation Plan; o Annual Performance Report (APR) due 120 days prior to the end of the budget period. This report must include performance measures and updated measures if needed, and evaluation results for the work completed to date; and o Performance Measure Report (PMR) – CDC may require more frequent reporting of performance measures than annually in the APR. When this is the case, CDC will specify reporting frequency, data fields, and formatting at the beginning of the award period. • Maintain a secure online platform for data collection and management • Collaborate with CDC project officers and CDC evaluators on ways to collect information to help monitor progress, communicate results, and identify technical assistance needs • Participate in monthly CDC and/or Agency webinars, conference calls, and meetings.
