Specifications include, but are not limited to: 4.2. Pre-Employment Examinations for TUSD Employees 1. Pre-Employment physical examinations are conducted pursuant to TUSD policy and American’s with Disabilities Act (ADA) regulations. The Chief Human Capital Officer, or designee, will be the Contractor’s liaison for the pre-employment physicals. The Contractor shall provide, or cause to be provided, medical examinations and related services to persons who are referred by TUSD. The contractor shall perform preemployment medical examinations to include: 2. Review TUSD Health History Questionnaire and the essential job functions of the TUSD position. 3. Conduct a physical examination as per TUSD form. 4. Provide a medical evaluation and report any restrictions or limitations for the performance of the essential job functions to be assigned in accordance with the Job Description and ADA regulations. 5. Laboratory work to include a urinalysis, simple (sugar, albumin, Sp. Gr.). 6. Additionally, a lift test and back strength/flexibility exam for medical categories A and B as defined by TUSD. The Contractor shall follow the guidelines of the lift test and back strength/flexibility form provided by TUSD. 4.3. Pre-Employment Drug Testing 1. The Contractor shall preform regulated pre-employment drug screen to include a 9-panel Non-DOT or 5- panel DOT Drug Screen. 2. The Contractor shall assure that the collection process is performed in accordance with professional medical standards and includes all aspects of the DOT and AZ DPS requirements. 3. Testing shall include, but not limited to, providing supplies and materials, collections sites, specimen collection, initial drug and alcohol screens, confirmation of drug and alcohol tests for positive initial tests, transportation of specimens, medical examination report, reporting and records retention and expert testimony services.
