Specifications include, but are not limited to: A. Shall provide a Kentucky Licensed Medical professional to perform general health physical examinations to the Kentucky Education and Labor Cabinet, Department of Workplace Standards OSH Program employees, according to the protocol outlined in the Attachment C “MEDICAL EXAMINATION PROGRAM,” Form 179. The vendor will migrate Attachment C to an electronic medical record that will be completed by Kentucky Education and Labor Cabinet, Department of Workplace Standards employees prior to their examination. The content in Attachment C shall remain the same, but the layout may differ when the transition to an electronic medical record is complete. The physical examinations are preventive in nature and related to the duties and exposures that the program employees might encounter when inspecting workplaces in all types of industry. The employees may be exposed to all the different hazards which occur in industry, such as excessive noise, silica, asbestos, lead, chlorinated hydrocarbons, solvent vapors, chlorine, ammonia, and various carcinogens. The vendor will attempt to honor requests for specific providers from the Kentucky Education and Labor Cabinet, Department of Workplace Standards but reserves the right to schedule from among its qualified providers. The Kentucky Education and Labor Cabinet, Department of Workplace Standards, reserves the right to reschedule. B. Must Provide physical examinations at the Vendors facility, available Monday through Friday from 8:30 am EST – 4:30 pm EST. Individual appointments will be set at mutually agreed upon days and times by DWS OSH staff, DWS OSH employee(s) and Vendor. C. All medical examinations must be conducted by a physician licensed to practice medicine in the Commonwealth of Kentucky, a certified nurse practitioner, or a certified physician’s assistant if under the direction of a physician licensed in Kentucky. D. Shall include at least the tests in the physical health examinations that are specified in Attachment DMinimum Medical Examination Tests. E. Shall provide the written results of the physical examinations, containing a health status report with special attention being paid to the following: 1. Any medical conditions which would be aggravated by certain kinds of workplace exposure (e.g., exposure to isocyanides, climbing ladders, etc.), such as those outlined in the document entitled “EMPLOYEE PHYSICAL REQUIREMENTS and POTENTIAL HAZARDS,” Attachment B. 2. Lung conditions which would contraindicate the use of respirators. The written report to the Kentucky Education and Labor Cabinet, Department of Workplace Standards, should enable management to monitor employees’ exposure to hazards which he or she encounters in workplace settings. All reports are due thirty (30) business days from date of exam. Other findings from the examination which are non-employment related shall be reported to the individual in private, if necessary.
