Specifications include, but are not limited to: Vendor shall deliver Clinical System Technical Assistance Package to include the following components: a) Identify and recruit three to four (3-4) Federally Qualified Health Centers/community clinics in Los Angeles County that are interested in receiving technical assistance to identify and address health care disparities and cardiovascular health outcomes. Federally Qualified Health Centers/community clinics should be selected in coordination with DPH and target those serving selected geographic regions in Los Angeles County (e.g., Skid Row). b) Facilitate regular (e.g., monthly) technical assistance meetings with each participating Federally Qualified Health Centers/community clinics to assist with key subject areas including – using electronic health records to track clinical and social needs and support team care; adoption of social services referral platforms; assembling multidisciplinary teams; creating communityclinical linkages; and remote patient monitoring tools for self-measured blood pressure monitoring. c) Facilitate and host 2-4 trainings (approximately 60 min each) for participating Federally Qualified Health Centers/community clinics on the use of standards process or toll such as Geo-mapping tools, to identify patients’ social service and support needs and monitor and asses the referral and utilization of those services. d) Engage in DPH’s Cardiovascular Health Learning Collaborative to identify new opportunities to support federally qualified health centers/community clinics receiving technical assistance. e) Develop a summary report of key findings based on information gathered through technical assistance efforts.
