Specifications include, but are not limited to: III.A Program Requirements A. Goals shall include, but not be limited to: A. Support the City in responding to referrals of non‐violent mental health crises. B. Help identify needs of Milpitas’ unhoused residents. B. Objectives shall be achieved by: A. Conducting Vulnerability Index‐Service Prioritization and Decision Assistance Tool (VI‐SPDAT) intakes and assessments in accordance with the current Homeless Management Information System (HMIS) standards for the County; A. Utilizing a multidisciplinary team approach; B. Developing a robust knowledge of community resources; C. Coordinating services with the services that the City provides for their homeless population. C. Outcomes to be achieved: A. Engagement and outreach to all individuals referred by the City as well as proactive outreach to individuals on a weekly basis. A. Number of referrals made; B. Follow up as needed on referral sites for mitigation; C. Number of clients from Milpitas and other locations; D. Increased self-sufficiency of homeless individuals E. 75% of individuals contacted who are not already enrolled in the Homeless Management Information System (HMIS) will have the VI-SPDAT administered; F. Link 75% of individuals with Severe Mental Illness (SMI) to Behavioral health Services Department (BHSD) HEAT clinical team for further engagement; G. 20% of clients shall be connected to medical services; H. 20% of clients shall improve housing situation; I. 80% of referrals will have attempts at initial contact within 2 working days. III.B Reporting and Data Collection Requirements A. Vendor shall serve a minimum number of 50 individuals per calendar year. B. Vendor to bill City on a quarterly basis C. Vendor may be asked to attend City Council meetings or meetings with City staff throughout the length of the contract
