Provide athletic training services to all eight (8) high schools within Granite School District. Nothing in the resulting contract(s) shall be construed as a requirement for student athletes to use the Contractor’s clinic or care facilities owned by the Contractor, or to any other company with which the Contractor has business relationships, nor does it obligate district employees, coaches, or ATCs to make referrals to the same establishments. Provide ATC service at 30, 40, or 60 hour per week options as selected by school for the term of the contract. These hours will include the required game attendance and remaining hours will be used by the school as needed. Provide an ATC at all home UHSAA athletic events giving priority to higher risk of injury sports. Due to the risk of injury football varsity away games will be covered as well. Additional athletic events that fall outside of UHSAA sponsored athletics can be scheduled inside remaining weekly hours. Any hours, as requested by the school agreed upon by both parties, beyond the contracted weekly hours will be reimbursed by school at the contract hourly reimbursement rate. Contractor agrees to provide athletic training therapy and medical services to school and its athletes. The ATC, Principal, and Athletic Director will determine the ATC’s on-site schedule at the school. ATC services are to be made available Monday through Saturday as need by the school and as agreed upon between contractor and school annually. The provider will conduct concussion baseline testing to all athletes requested by school. The provider will manage all return to play protocol for athletes suffering from concussions. Provider must indicate which of the District’s high schools they are interested and capable of providing services for. The district has eight high schools; Cottonwood, Cyprus, Granger, Hunter, Kearns, Olympus, Skyline, and Taylorsville. Contractors may bid for all schools or for some of the schools, but the services offered to any one school by the Contractor must be the same for all schools they are bidding and potentially awarded. Schools and Contractors may not negotiate different terms to any awarded contract. Contractor and its employees will be considered independent contractors and are not employees of the school district. Contractor will maintain adequate malpractice liability insurance for their ATCs and shall provide proof this insurance in their proposal. Contractor will ensure that ATCs licensure is current in the State of Utah, as well as fingerprint and criminal background check meeting the requirements of UCA 53G-11-402. District Responsibilities The school will provide an annual budget for necessary supplies for use by ATC at a negotiated minimum. Provide space for therapy and medical treatment area(s).
