Specifications include, but are not limited to: 4.1.1 Prior to placement in the Facility, Vendor shall perform, at vendor’s cost, for the Dietitian: 4.1.1.1 Background check through WV Cares 4.1.1.2 Twelve-panel drug screening 4.1.2 The following information must be submitted to the Agency before the Dietitian reports to work: Completed application or resume, at least three (3) job references as proof of experience, copies of licenses and registrations, negative drug screening, and WV Cares background check. 4.1.3 Agency reserves the right to refuse any staff member and/or to request removal and replacement. 4.1.4 Successful vendor must incorporate all anticipated pre-employment expenditures, administrative and overhead costs, taxes, FICA/FUTA, travel expenses, incentives and any other cost incurred by vendor into their all-inclusive hourly rate for each position and shift. Agency will not pay any fee separately from the hourly rate. 4.1.5 Vendor must review and evaluate all new admissions no later than 72 hours after admission per regulations. 4.1.6 Vendor must be available for consultations at the request of the Agency. 4.1.7 Vendor must monitor and evaluate nutritional assessment needs and internal feedings calculations of all tube feeding residents at least monthly and document all recommendations/changes in resident’s charts. 4.1.8 Vendor must document all findings/recommendations in resident’s charts.
