1. All work shall be completed by a certified NM Petroleum Storage Tank Bureau (PSTB)-approved contractor. 2. The specified work shall be completed at each of the two locations listed below. a. City of Farmington Municipal Operations Center (MOC) Bulk Fuel Site 101 North Browning Parkway, Farmington, NM 87401 b. City of Farmington City Hall Bulk Fuel Site 900 North Municipal Drive, Farmington, NM 87401 3. There are ten UDC’s at the MOC location, each having a double-wall steel line supplying the dispenser. 4. The City Hall site has two UDC’s, each having a fiberglass line supplying the dispenser. 5. Icon Protex SplitRepair Flange Fittings or approved equal are required. All labor, equipment, and materials necessary are to be furnished by the awarded contractor. 6. If the dispensers must be removed to complete the task, they shall be returned to their original position and function upon completion of each sump. 7. The awarded contractor will remove the old boots and install the replacement boots in each of the 12 UDCs. 8. The UDCs shall also be inspected and tested for any leaks not related to the fuel line boots. Any problems found shall be reported immediately to the City’s Class A/B operator. Additional repair work shall be quoted separately from this RFQ. 9. Except for unrelated leaks as defined in paragraph V. 8., the contractor shall supply a passing NM PSTB certified test of each sump upon completion.
