Specifications include, but are not limited to: The proposal should consist of the following minimum services as part of a pre-employment program for the Town of Farmington: 1) Police employees • Screening history and comprehensive physical examination • Blood test including chemistry, complete blood count, and hepatitis panel • Urinalysis • Audiometry and vision screening • Exercise stress test • Spirometry with respirator certification • Optional chest x-ray (per physician’s discretion) • Hepatitis B vaccine and tetanus booster (as needed, once employed) • Drug Screening for controlled substances, including anabolic steroids. • Tuberculin skin testing • EKG; 2) For office workers • Screening history and intermediate physical examination • Drug screening; 3) For public works employees • Screening history and intermediate physical examination • DOT (NIDA) urine drug test (pre-employment only) • Audiometry; 4) For water pollution control plant (WPCP) employees • Screening history and intermediate physical examination • Spirometry with respirator certification • DOT (NIDA) urine drug test (pre-employment only) • Hepatitis B, diphtheria and typhoid shots; 5) Firefighters (full-time and volunteer) • Screening history and comprehensive physical examination • Spirometry with respirator certification • Blood test including chemistry, complete blood count, and hepatitis panel • Urinalysis • Audiometry and vision screening • Exercise stress test (full-time mandatory, volunteer per physician’s discretion) • Optional chest x-ray (per physician’s discretion) • Hepatitis B vaccine & Booster/Titer • Tetanus booster (as needed, once employed) • Drug screening • Tuberculin skin testing • EKG • Submission and Processing of required annual OSHA questionnaires per 29 CFR 1910.134 via mail between firefighter and medical provider or via an online secured system/platform.
