1) HCCCP Coordination. a) Coordinate, implement, and participate in (in-person or virtual) meetings, which may include: i. Executive Committee (monthly) ii. Steering Committee (quarterly) iii. Quality of Life Action Team (monthly) iv. Vaccine Preventable Cancer Workgroup (as needed) v. Colorectal Cancer Task Force (monthly) b) Schedule and plan in-person or virtual meeting dates/times. c) Develop meeting agendas. d) Facilitate and/or co-facilitate and participate in the meeting (in-person or virtual) discussions with the Cancer Programs Director (“CPD”) and meeting participants. e) Take meeting minutes and disseminate as appropriate and ensure completion of action items. 2) HCCC Event(s) Coordination. a) Convene planning committee(s). b) Coordinate logistics and related event activities. c) Develop themes and agenda(s). 3) HCCCP Implementation. a) Coordinate and implement HCCCP Year 2 work plan activities. b) Implement activities to increase breast, cervical, and colorectal cancer screenings by collaborating with Action Teams, Workgroups, and community partners including the CDMB, Bilingual Aide Section. 4) HCCCP Collaboration. Participate in weekly meetings (in-person or virtual) with the HCCCP team to ensure progress toward completion of deliverables. 5) Reporting. Meet all reporting requirements as determined by the CDMB, including: a) Submit a monthly summary to the CPD no later than the seventh day of the following month. Summaries shall include the following information for the services performed during that month: i. Status of meeting/event(s) coordination. ii. If applicable, list of confirmed meeting(s)/event(s) dates. iii. Summary of the progress of work plan activities. iv. Barriers encountered and the resolution of the problems. v. Plans for the next month, including brief narrative explaining the projected activities. b) Assist the CPD in writing annual performance report and evaluation report.
