Specifications include, but are not limited to: a. Required: License, Insurance, Prevailing Wage, etc. b. Inspector must be CCCDI (Cross-Connection Control Device Inspector) c. Contractor must provide their own aerial lift and or ladders as needed to complete all testing. d. Contractor must have their own vehicle, tools and materials. e. Material markup rate not to exceed 7%, and any other fees and conditions or documents associated with these required annual inspections. f. It is the responsibility of the contractor to verify the device counts at each site and to supply the district with an inventory report by location. g. Upon completion of these inspections, a Backflow Inspection Test Report will be sent to School District U-46 (Plant Operations) showing when they were tested last. The Backflow Report must be left on-site by the inspection device at each location and a copy needs to be sent to the townships and or villages or as the Illinois Plumbing Code 890.1130 Protection of Potable Water requires. h. Upon completion of the inspection at each site, the vendor will submit a detailed quote for repairs that are needed. The district will review for possible approval to move forward with the repair. If the district approves the repair the vendor will be required to complete any building permits, if required, to complete the repair. i. Dedicated contact for communication via text, email and phone. j. Full invoice required for all repairs, with a cap on materials not to exceed 7% over cost. k. Itemized billing per completed job.
