Specifications include, but are not limited to: Constructing comprehensive discharge plans • Assess referred gang-involved individuals’ criminogenic risks and needs and most pressing needs. Work with them to develop individualized discharge plans that address short and long-term needs upon reentry, including specific steps and activities to manage them. These areas include but are not limited to, gang desistance planning, housing, finances, employment, physical health, mental health, substance abuse, education and training, parenting and family, and legal. • Develop a professional alliance, assist individuals in completing applications for services, government documents, and benefits such as Medicaid, Medicare, SNAP, SSI, and Current Care, and provide information about available resources. Referring to community-based services • Refer individuals to appropriate community-based services to address their needs in preparation for their discharge from the RIDOC. Community-based services targeted for referral should include, but not be limited to, housing, finances, employment, physical health, mental health, substance abuse, education, training, parenting and family, and legal requirements. • Communicate with agencies and providers to refer offenders ensuring that the referrals result in service provision and/or providing information about eligibility and application processes. • Assist individuals in scheduling, planning, and preparing for appointments.
