Specifications include, but are not limited to: Multnomah County Health Department's Healthy Families program seeks proposals for comprehensive home visiting services targeting parents of color & families experiencing higher stressors & challenges. Major service components include: • Management of incoming referrals to the Home Visiting Team • Assessments for maternal depression, family safety, child development and parenting stress • Development of a unique plan for each family to be carried out by their Home Visitor, addressing identified needs and risks • Home visits (and optional group events and outings) • Coordination of services with other programs serving the family in the home (Child Welfare, Early Intervention, Substance Use Disorder programs, Mental health services, DV services, etc.) • Coordination of HV services with the families’ “medical home” provider (i.e. primary care doctor) • Parent education using approved curriculum • Child development assessment and intervention • 1.5 hours of Weekly one-on-one Reflective Supervision of all Home Visitors Additional elements to be addressed by all Home Visiting Teams include: • Assurance of Confidentiality • Child Abuse Prevention and Reporting • Adherence to the Healthy Families America model • Cultural humility and responsiveness • Program Administration • Robust data collection
