Specifications include, but are not limited to: 3. SCOPE OF WORK 3.1 The Contractor must provide assessment and psychological intakes for referral victims of crime. 3.2 The Contractor must provide individual, couple, family and group counseling to adult and child survivors of physical, emotional or sexual abuse. 3.3 The Contractor must write treatment plans, progress notes, discharge summaries and other chart entries as required by agency policies and procedures, licensing agencies, funding sources and ethical standards. 3.4 The Contractor must provide all services in compliance with agency policies, federal and state laws, professional ethical standards and behavioral health licensure regulations. 3.5 The Contractor must follow guidelines for accurate data collection and reporting as required by agency, funding source and referral provider. 3.6 The Contractor Must adhere to agency requirements of confidentiality, professional conduct, personnel, financial and clinical policies, supervision, training and job qualifications. 3.7 The Contractor must attend and participate in appropriate clinical supervision on a weekly and/or monthly basis. 3.8 The Contractor must participate in muti-disciplinary team meetings as appropriate. 3.9 Contractor services may need to be provided onsite at the Glendale Family Advocacy Center, Glendale, AZ. 3.10 The Contractor must be willing to meet once a quarter for a quality assurance meeting with the Victim Assistance Supervisor. 3.11 Contractor shall provide demographic statistics (quarterly) on the victims served to meet Federal grant requirements. 3.12 Contractor shall track/document service hours and provide this documentation for statistical purposes.
