Lane County Department of Health & Human Services is seeking information on behalf of the Community Health Centers of Lane County (CHCLC) which is a part of the Federally Qualified Health Centers (FQHC). The Community Health Centers of Lane County (CHCLC) serves approximately 25,000 patients across the Eugene/Springfield metro area in Lane County, Oregon at six different clinic locations. Two additional clinic locations are slated for opening in 2024, one located in the neighborhood of West Eugene and the other in Cottage Grove, Oregon, a rural community located 22 miles south of the metro area, with an approximate population of 10,000. CHCLC believes healthy people are the foundation of healthy communities. CHCLC offers compassionate care for infants, children, adolescents, adults and seniors. As a safety net provider, CHCLC serves members who are covered under Medicare/Medicaid, unhoused or under/uninsured, who require a range of acute, chronic and preventive medical care services. Other services offered include reproductive health, pediatrics, dental prevention, integrated oral health, whole person integrated behavioral health services, and alternative care.
