The Minnesota Department of Human Services (DHS), through its Aging and Disability Services Division and the Age Friendly Minnesota Council (STATE), is seeking Proposals from qualified Responders to develop and/or provide supported decision-making programs to Minnesotans to reduce or avoid more restrictive forms of decision making, such as plenary guardianship or conservatorship, and determine if expansion of accessible supported decision-making services will help “people in Minnesota thrive” as stated in the DHS Agency Strategic Plan 2023. This funding will award general operating and technical assistance grants to public and private nonprofit organizations, counties, and Tribes, to provide and promote supported decision-making. Proposals may include provision of services to an individual, family members, or trusted support people of the person needing services; promotion and advancement of supported decision making; and development of new programs to expand the education and implementation of supported decision-making programming in racial and geographically diverse populations. The term of any resulting contract is anticipated to be for up to thirteen months (13 months), from June 1, 2024 until June 30, 2025. STATE may extend the contract up to a total of five (5) years.
