The STATE is seeking an organization to manage all aspects of the customized living provider transition grants. The STATE is seeking a Responder who will: Develop an application process that allows eligible service providers to apply for transition grants up to $50,000 per setting. The application process must close on June 30, 2024. Use information provided by STATE to determine eligibility of each service provider and setting that applies for the grant. Negotiate grant contracts with eligible service providers. Complete provider site visits to confirm grant contract work is completed as described. Pay contracted providers according to terms identified in each grant contract. Provide quarterly reports to STATE that include critical information related to ongoing administration of the transition grant program. As part of the transition grant program, the STATE will: Work collaboratively with the selected Responder to ensure contract requirements are met. Provide information to the selected Responder to assist in determining service provider eligibility for transition grants. Provide guidance to the selected Responder if questions related to administration of the grant program arises. Review reports submitted by the selected Responder.
