The Contractor will finish out the end of Year 1 adding to the annual Evaluation Plan and building on the data collection from the previous evaluator as indicated below. The Contractor will be responsible for highlighted reports using Y1 data that are due at the start of Y2. In the remaining years of the grant, the contractor will provide Agency evaluation services and deliverables as described and specified below : 1. Provide evaluation services to the Program: • Address the following Program evaluation questions: o To what extent is a strong school health infrastructure in place at the state, LEA and school level to support improving the health and well-being of students? o To what extent are LEAs and schools prepared to implement evidence-based school health policies, practices, programs, and services? o To what extent have priority LEA and schools developed school health action plans based on assessment results and school priorities? o To what extent are LEAs and schools implementing evidence-based school health policies, practices, programs, and services? o To what extent has health equity been promoted among LEAs and schools? o What is the overall impact of this initiative on the health, academic achievement, and well-being of students? • Report on evaluation results and performance measures annually; • Report on Program successes and challenges; • Report on indicators and outcomes related to PD, TA, and action plans; • Report on indicators and related measures on the implementation of strategies; • Report on indicators and related measures on the impact the activities on short-term and intermediate outcomes; • Report on accomplishment, facilitators and barriers related to achieving the overall goals and objectives; • Provide recommendations on Program implementation and continuous program quality improvement; • Perform qualitative data collection and analysis with individuals from the priority LEA; and • Conduct statistical analysis on performance measures and project outcomes.
