Specifications include, but are not limited to: 1. Provide a person-centered, culturally sensitive independent living skills training program to adults with severe or intractable epilepsy. 2. Provide person-centered information, assistance, and system navigation to parents and/or guardians of children, birth to age 18, who have been diagnosed with intractable epilepsy. 3. Conduct outreach to underserved populations. In an effort to provide, “locally focused”, “individually centered”, and “easily accessible” services, conduct outreach activities to historically underserved populations and/or communities where health inequities exist e.g. People of color, American Indians, Asian, Latinx, people with disabilities, people living in poverty, members of the LGBTQ community, and people with mental health conditions. 4. Program objectives include: a. Assist service recipients to increase their problem solving and interpersonal skills; b. Assist service recipients to increase their independent living skills in the areas of communication, stress management, goal setting, money management, and medical self‐management; c. Assist service recipients to acquire and maintain apartments or homes of their own; d. Assist service recipients to identify and access services and supports for which they may be eligible; e. Help persons to achieve independent living. Service recipients who are eligible for Community Access for Disability Inclusion/Brain Injury (CADI/BI) waivers will be transitioned to receive services under those waivers; f. Assist service recipients to obtain and maintain a support system in the medical community; g. Assist with education, job training, and obtaining employment as related to the goals for independence in the person’s individual training plan (ITP).
