Specifications include, but are not limited to: The State of Kansas is seeking competitive bids to provide Minimum Data Set (MDS) services for the Kansas Commission on Veterans Affairs Office – Kansas Soldiers’ Home, per the following specifications. 4.1. MDS Management • Work with team to schedule, plan and manage MDS completion 4.2. MDS Completion • Review and ensure MDS’s are completed timely by all the IDT • Set ARD’s and complete clinical items 4.3. CAA/Care Plan • Complete Care Area Assessment for items triggered by MDS • Ensure each participant completes there triggered CAAs • Provide RN signature of CAA Completion • Drafted Comprehensive Care Pan with admission MDS i. Baseline care plan is not included 4.4. Case-Mix • CMI management to identify and capture planned or current resources • Schedule ARD’s 4.5. Daily Resident Review (Direct Communications) • PDPM Review and Management • OBRA/LTC Review and Management • ICD10 Coding for admissions • Daily Chart Review for resident changes and management • Daily Orders Review for resident changes and management • Care Plan review for CAA Information or other concerns • QIES ASAP Reports and Management • Survey Assistance for MDS • Medicare Hotline Long Term Care questions and answers 4.6. Audits Include: • High level QM reviews Quarterly • PDPM Financial updates/CMI reviews • Random Claims review to identify education needs or trends 4.7. Education • SSD/SS designee MDS/CAA education on request • Dietary/designee MDS/CAA education on request • Activities/designee MDS/CAA education on request • BOM/Designee MDS/PDPM review for billing on request • PDPM/Case Mix facility training (putting it all together) on request
