Specifications include, but are not limited to: 2.1 Practice and specialize in geriatrics and/or long-term care and be current in the speech therapy field. 2.2 Be available to Home a maximum of three (3) hours per month to perform the consultation duties identified in this contract. 2.3 Coordinate a schedule of availability with the Home for timely evaluation and individualized treatments for qualifying residents in accordance with Part A & Part B Medicare regulations. 2.4 Coordinate provision of services under this contract with the Director of Nursing at the Home. 2.5 Provide monthly reports of services performed. Meet at least quarterly with the Home to evaluate services provided. 2.6 Plan and conduct in-service training programs for staff of Home, as requested by Home and as related to Contractor’s profession. 2.7 Possess current, valid Minnesota license to perform services under this contract and keep licensure current during the term of the contract. Responders shall provide evidence of licensure with their proposal. 2.8 The chosen contractor shall ensure they are free from communicable diseases as required by Minn. Stat.§ 144A.04 and Minnesota Administrative Rule 4658.0805. 2.9 The chosen Contractor shall provide Minnesota Department of Human Services Background Checks attesting that all persons performing services under this contract are not disqualified from providing direct care services to residents of nursing care facilities, pursuant to Minn. Stat. §144.057 and 245A.04. Contractor understands and agrees that no services shall be performed by persons who have been disqualified from providing direct care services to nursing care facilities, pursuant to the above-references statutes.
