The District is seeking proposals from firms to provide the Employee Assistance Program (EAP). The scope of work includes: 1. Provide benefits and/or benefit related services, which match to the current plan for employees and their dependents 2. Adhere to industry standards in the delivery of benefits and services 3. Provide a designated Account Manager and Account Management Team 4. Provide issue-specific face to face counseling services to employees and eligible dependents through professional counselors 5. Provide a minimum of three (3) face-to-face visits per issue, per employee or eligible household member each calendar year 6. Provide 24-hour per day, 365-day per year toll-free telephone crisis counseling 7. Provide referrals to qualified, behavioral health providers in the District’s medical plan network upon request of the employee or eligible dependent requiring continuing care following the initial three visits 8. Provide four hours of critical incident stress management services per year 9. Provide a website for use by employees and eligible dependents to obtain information on the program including behavioral health and wellness information 10. Provide quarterly statistical reports of program utilization to the District 11. Provide communication strategies and materials that include at a minimum employee brochures and supervisor manuals 12. Provide electronic promotional materials and flyers in English and Spanish 13. Provide exceptional customer service support via phone, e-mail and website 14. Provide a minimum bank of 4 hours to be used for Orientation/Training/Educational Seminars/Critical Incident Stress Management 15. Provide Web-based WorkLife Services, Legal and Financial Services and Identity Theft Services 16. If requested by the District, attend Health/Benefit Fair (1 Fair per year, as needed) 17. If needed, Mediation for HR Referrals
