Specifications include, but are not limited to: The District is seeking proposals from firms to provide Fully-Insured PPO Dental Services. The District wants you to match the current plan designs as closely as possible. The following components should be included in your response to the RFP: 1. Provide a designated Account Manager and Account Management Team 2. Provide members with adequate network access through a single network or dual network approach 3. Provide a designated call and claim unit with toll free telephone access. 4. Provide customer service to answer inquiries on claims, eligibility, provider network, services, coverage, or other inquiries from participants Monday through Friday from 8:00 AM to 6:00 PM (AZ time) 5. Attend Annual Open Enrollment Meetings as requested by member districts, during normal business hours 6. Provide bilingual communication/educational materials 7. Coordinate and mail ID cards and policy summary/certificates to participants’ homes, (as applicable). 8. Provide claims forms (as applicable) 9. Receive claims and process payments of benefits in accordance with the plan designs for all claims incurred 10. Correspond with participants and providers if additional information is necessary to complete the processing of claims 11. Determine benefits payable under the Plan, pursuant to the terms and conditions of the District’s Benefit Plan booklets 12. Coordinate benefits payable under the Plan and with other benefit plans, if applicable
