Specifications include, but are not limited to: a) Organize, conduct, and synthesize 12-15 stakeholder interviews (to be held virtually) with HBLA Coalition members to gather input needed to develop an implementation plan. b) Expand the HBLA Coalition to include a Community Advisory Group (CAG) with at least 7-10 community members from priority populations and persons with lived experience with dementia/dementia caregiving who will provide feedback and guidance on the implementation plan and support Coalition activities. c) Plan and facilitate 2-3 CAG meetings. Meeting goals and agenda to be developed in collaboration with DPH. d) Plan and facilitate 3-4 virtual meetings with the HBLA Coalition to develop an implementation plan. e) Develop an implementation plan and summary report synthesizing lessons learned based on information gathered at interviews and meetings.
