Specifications include, but are not limited to: Provide hazardous materials clean up and restorative measures of UDOT ROW and the environment to include containment, removal, and proper disposal of contaminated materials such as fuel, oil, chemical, and tanker spills. Work with UDOT encroachment permits as needed prior to beginning work. Provide UDOT with a proposed work plan on how the site will be remediated. Provide traffic control as required by UDOT standard specifications. Resupply Fire Departments with any materials utilized for clean-up and containment activities prior to Contractor’s arrival (peat moss, booms, kitty litter, poly pads, as an example). Provide and maintain all needed BMPs throughout the duration of the project. Contact and coordinate with the local Health Department and all other required environmental regulatory agencies. Comply with all local, State and Federal environmental laws, ordinances, and statutes. Contractor will adhere to a NO CHASE policy. Contractor will only respond when called by UDOT or when selected by the Responsible party for hazardous material recovery and clean up. Contractor will provide the UDOT with an annual report which includes but is not limited to: number and type of calls for service; summary of charges per call; report of customer complaints and/or billing or cost disputes. Annual reports are due June 30th following each contracted year of service Contractor will provide UDOT, the local Health Department, and Utah DEQ all documentation related to the cleanup provided within 30 days of the completion of the remediation. Documentation may include testing results for soils if affected, invoices, photos documenting before and after cleanup, proof of proper disposal, and any other records relating to the cleanup provided. Contractor agrees to complete a UDOT Waste Disposal Tracking Form when disposing of any material.
