Specifications include, but are not limited to: The purpose of this contract is to formalize an agreement between ADVS and Contractor to provide Case Management Services to the Transition in Place Program (TIP). The Program shall be used to place eligible veterans experiencing or at risk of experiencing homelessness in housing and with concurrent supportive services using the Arizona Military Family Relief Fund (MFRF). The Contractor shall: 1.1. Have the knowledge, skills and ability to appropriately house and provide case management for homeless Veterans. 1.2. Maintain and utilize a policy and procedure that includes, at a minimum, detailed intake procedures, program description and eligibility requirements, nondiscrimination policy and confidentiality requirements. 1.3. Verify and assure that staff members do not have conflict of interest in the provision of services and management of the programs. 1.4. Utilized a computer-based tracking system to generate reports on a monthly, quarterly, and any other reports. 1.5. Provide all staff appropriate training for services provided by the contractor in a timely and accurate information. 1.6. Maintain client confidential information in a secure location. 1.7. Identify and appropriately address social determinants of the Veterans enrolled in the program to include but not limited to: housing, food stability, psychological care, social support systems, spirituality, etc. 1.8. Provide services with one (1) full-time dedicated Case Manager for every twenty (20) homeless Veterans to assist with connection to housing, military resources and wrap around support. 1.8.1. Provide additional Case Managers if the number of homeless Veterans increase past twenty (20) program participants. 1.8.2. Provide a daily staffing report on MFRF families upon request. 1.9. Reserve the right to terminate individual case management services at any time, as well as the Veteran. 1.9.1. ADVS must be notified of all requests for termination of individual case management within twenty-four hours (24). 1.9.2. Contractor must receive approval from ADVS for any program participant discharges from the program. 1.10. Availability: 1.10.1.Be available during State business hours from 8:00 AM – 5:00 PM. Five (5) days a week Monday through Friday; 1.10.2. Respond to ADVS and program participants inquiries within twenty-four (24) hours; 1.10.3.Submit Contractor’s contact information (phone and email); and 1.10.4.Participate in bi-weekly face-to-face meeting the ADVS staff.
